Fees and Payment Policy

For Patients Without Insurance

In an effort to keep dental costs down while maintaining a high level of professional care, we have established the following payment plans for the use of our patients. Fees may be paid as follows:

1. For extensive treatment plans, prepayment, or payment at the time of treatment with a 5% discount.
2. Payment of balance in full at time of treatment.
3. Use your VISA, MASTERCARD, Discovery or Debit Card.
4. Extended payments are available with special arrangements before treatment. Accounts outstanding more than 60 days from treatment date will bear interest at 1.5% per month, 18% per annum.
5. All major treatment and treatment involve a laboratory procedure will require an appropriate down payment.
6. Our senior citizens over age 65, and without insurance, will receive a 10% discount for payment at time of treatment.

Emergencies

If you have a dental emergency, call the office immediately, day or night. The number is 636-391-9904.

Daytime Emergencies
If it is in the daytime, we will do our best to work you in as soon as possible. Call 636-391-9904.

NIghttime or Weekend Emergencies
If your emergency is at night, please call the office and follow the instructions on the message machine. Call 636-391-9904.

The machine will provide exact instructions on the most expedient way to contact Dr. Gross or a member of our staff who can connect you with a dentist on-call.

Dental Insurance

For Patients With Dental Insurance

Or "Dental Assistance", as it should be called, is designed to help pay part of the cost of dental treatment. Your employer has made this coverage available to you and we will do our best to help you maximize its benefits. Dental insurance is not designed to pay all of the cost of treatment, but rather to be a partial aid.

Your insurance contract is between you and your insurance company. The type of benefits in your contract depend on what your employer negotiated with his company and the amount of money you wish to pay in premiums. We ask that you assign your insurance benefits to us.

1. We will bill insurance companies for check-up visits.
2. For all other treatment, because varying deductibles apply, patients must pay 20% of the fee at the time of treatment. We will file insurance for all procedures.
3. In cases of extensive treatment involving fees over $100, we will submit a "Predetermination of Benefits" to your insurance carrier and work out financial arrangements with you.
4. Please help us to help you gain maximum benefits by bringing your insurance benefit booklet. If an insurance payment varies from the total fee charges, an adjustment will be made.
5. All PPO copayments must be made at the time treatment is rendered.
